This aircraft was scheduled for a flight to Germany and prior to fueling, a fuel test cell check was performed. Non-standard plugs were used in the vent lines and when the aircraft was released for fueling, one of the non-standard plugs was left in place. During refueling, 3 of the fuel tanks in the left wing were pressurized beyond tolerance and ruptured. Six people were injured.
